Supplies Sheet

All Supplies Invoices must have original receipts attached when submitted.

Please ensure that receipts do not include any personal purchases and that the total value of the receipt is for CREATE supplies only.

Your total budget for supplies will be determined at the beginning of the school year and will reflect the available funds in the program budget.

If you are a Visual Artist your budget will vary depending on the number of classes you have been assigned per term.

If you exceed your allotted budget you will not be reimbursed.
